Comprehensive Initial Psychiatric Evaluation
60 minute session - $500
This is a comprehensive evaluation which will include discussions on the current illness, medical & social history, laboratory testing and treatment recommendations
Comprehensive Sports & Performance Initial Evaluation
90 minute session- $600
This entails a Comprehensive Initial Psychiatric Evaluation and will include a deeper dive into performance history
Follow Up Evaluation
30 minute session - $250
During these visits we check patient’s progress and make adjustments to the treatment plan as needed
Sports & Performance Follow Up Evaluation
45 minute session - $375
Similar to the Follow Up Evaluation with extra time spent on analyzing performance and present barriers to further success
Insurance Policy
At this time, Dr. Fields is not accepting any insurance. We will provide you with a Superbill if requested which can be submitted to insurance for potential reimbursement.
Cancellation Policy
Please provide 24-hour notice prior to cancelling appointments. If an appointment is not cancelled within 24-hour notice, a charge the same amount of the appointment will be billed at the time of the appointment.
